Approaches to Address Disparities in Provision of Health Services – Social Determinants of Health Framework
Health promotion and disease prevention is integrated in all levels in order to address health disparities. In addition, there are other aspects that can be included at the other levels depicted in the diagram (for example: screening and diagnosing diseases, promoting healthy behaviors and assisting with changing unhealthy behaviors such as tobacco use, reproductive planning/pregnancy care, providing resources and assisting with accessing these to enhance health outcomes).
