  • Incorporates social determinants of health as part of the health assessment of:
    • Individuals
    • Families
    • Communities

The Maternal-Child Health Bureau supports approaches to maintaining and improving health outcomes using “whole-person, whole-family, whole-community” models. These models include the social determinants of health at all levels of the health care system.
